For international travel, the Downingtown Area School District, State of Pennsylvania, and EF Tours require several forms to be completed.

Most forms are electronic. After submitting, you’ll have the option to save a copy to your computer.

Please note: most forms are due on or before February 28, 2025, with the exception of the Authorization for Medication Administration, which should be submitted at the earliest convenience.
